The CTCC’s role in discharge planning aims for quality care and the identification of appropriate and timely services for progression of care and for post-acute discharges. The job responsibilities include proactive involvement in the IDT, screening for patients at risk for readmission with focus on disease management, completing PRI’s, Screens (Wakefield only), EDD oversight, managing SNF returns, addressing discharge sensitive issues, including discharge appeals, clinical input with post- acute authorizations, assisting with the care planning for the chronically ill and complex patients, and teaming up with nursing to make sure that patient education is done timely for discharges. The CTCC/CM will participate in the oversight of TB requirements for discharge.
Bachelors Degree required, Masters preferred
5 years experience
New York State License for Registered Professional Nurse and current registration
Case Management Certification preferred.
