This position, in collaboration with the Program Manager, Medical and Surgical Directors, cardiologists, transplant coordinators and
other health care providers will be responsible for assuring the development, implementation, and coordination of a plan of care for designated patients population through collaboration with interdisciplinary team members. The clinical care coordinator assesses plans and monitors care; identifies and manages variances: facilitates progress toward expected outcomes for the pre and post-transplant cardiac patient. Triaging patients and ensuring appropriate admission when applicable. Performing outpatient clinic assessments and transplant patient index discharge preparation. Ensures adherence to protocols and program certification requirements, quality assurance, and performance improvement. Responsible for maintaining the program in continued compliance with UNOS and CMS standards.



-Bachelor’s Degree in Nursing required- BSN.
-New York State Nursing License required.
-Five years of clinical experience in a direct patient care setting. Two of which must be in the specialty or related service.

-Clinical experience in inpatient and outpatient settings. Experience with care management of cardiac/transplant patients.
-Transplant experience preferred
-Effective English communication skills, both written and oral
-Commitment to interdisciplinary teamwork and continued professional development
-Computer skills including data entry, word processing, and electronic messaging including window-based software applications.
