LPN

This LPN will play a key role in the Medical Center's Outpatient Parenteral Antibiotic Therapy [OPAT] program. S/he will participate in performance improvement/research activities for the program, assist in maintaining a patient care database, assist withhome health, infusion therapy, ancillary and other outpatient follow up care in coordination with other members of the OPAT team. S/he will provide outreach to patients before [after the pandemic] and after hospital discharge for education and care coordination purposes. S/he also will effectively relate message information to the OPAT physicians or other providers for proper follow up and will document such follow up in the electronic medical record. May be required to function on various campuses and office settings. Must have a working knowledge of Microsoft Word, Excel and Power Pointl. Knowledge of Epic, the EMR, is a plus.

Job Requirements:
New York State license for Practical Nurse and current registration.
Effective written, oral and interpersonal communication skills. Bilingual in Spanish preferred.
Ability to enter and retrieve basic data from an electronic system or application.
Wound care experience a plus.
Ability to use printers, computers, fax, copiers and cell phones.
