Job Description: Bilingual Outreach Representative (English/Spanish)
The Montefiore School Health Program (MSHP) provides comprehensive medical and mental health services at 33 school-based health centers (SBHCs) located in Bronx, NY, and Yonkers, NY. A growing number of SBHCs also are providing dental, vision, and community health services. The Community Health Division uses a public health approach to eliminate health disparities facing student households we serve. The MSHP Community Health Division seeks to hire an Outreach Representative to facilitate work with school communities we serve.
Position Summary:
The School Health Outreach Representative (OR) will provide care coordination services and conduct outreach and health education to improve patient care and health outcomes with target populations. Outreach and education will be supported by virtual, telephonic, and in-person activities promoting the program and school-based health center (SBHC) services to youth, parents, key stakeholders, and school communities. The OR will coordinate and conduct health screenings, individual and group educational presentations, health fairs, health events, SBHC tours, engagement chats, and enrollment campaigns. The position includes encouraging the increase of reproductive health service utilization of high school students, including documenting every patient encounter, services provided, and outcomes in EMR and other program reporting tools. Screen for Family Planning Benefit Program (FPBP) eligibility and complete FPBP enrollment application and recertifications. ORs will also support outreach and recruitment for interdisciplinary health initiatives and studies to increase SBHC utilization, including data collection, entry, and reports. The ORs will travel to multiple sites as needed to support program deliverables. This is a grant-funded position.
Requirements:
• Bachelor’s Degree in Health Education, Psychology, Public Health, Human Service, or related field required.
• Bilingual in Spanish (written and spoken) required
• Able to travel to multiple MSHP sites as needed to meet the needs of the community
• Some evenings and weekends may be required to meet the needs of the community
Knowledge, Skills, & Abilities
• At least one-plus-years of relevant work experience
• Strong facilitation and public skills speaking experiences in presenting materials to small and large groups
• Comfort with delivering Reproductive Health Education (birth control, STI prevention, condom demonstrations)
• High comfort level in building and maintaining rapport with stakeholders and school community members
• Must have the ability to do data entry, use retrieval systems, and generate data reports (e.g., RedCap, EPIC, OSCR)
• Able to demonstrate leadership on projects by working independently
• Able to work within a team environment and work collaboratively to accomplish common goals and objectives
• Excellent written and verbal communication
