PATIENT EDUCATOR

BRONX, NY, US


Through a comprehensive strategy, provides leadership and support in developing health-promoting school environments to improve the well-being of all students and their communities. The Patient Educator is responsible for coordinating the effective delivery of classroom-, school- and community-based health promotion and preventive initiatives in cooperation with the school, community partners, government agencies and community members. On a classroom-, school- and community-level, the Patient Educator coordinates the implementation of MSHP developed and/or collaborative health promotion initiatives, conducts health education workshops, facilitates youth development groups, organizes outreach programs and needs assessments, and supports health policy development and implementation to improve student’s health and well-being.

Bachelor's Degree in Community Health Education, Health Administration or a related field required. Three years' experience. 

Two-three years' experience conducting health-specific programs and/or policy initiatives that involves children, adolescents, parents, and or one or more of the following constituents: school administrators, teachers, public health and medical professionals.

Leadership experience planning, implementing, and evaluating programs in the areas of health and wellness.

Proficiency in basic public health issues, methods, and principles.

Excellent written and verbal communication skills, including the ability to successfully interface well with a variety of stakeholders including parents, teachers, community members and leaders and students in grades K-12.

Strong facilitation skills and experiences in presenting training materials to large and small groups.

Experience organizing and conducting community outreach initiatives.

Computer literacy and proficiency in Microsoft Word, Excel, and Power Point.

Experience with program evaluation, research, and data collection.

Knowledge, understanding, and experience in two of the following priority areas: asthma, reproductive health, physical activity and nutrition.

Able to work evening hours and weekends for community health related events and programming.

Proficient in secondary language if host school's English Learners' population is greater than 40%.

Able to travel throughout the Bronx, NY, and the metro NYC area and work within NYC DOE public schools.
