The Patient Educator provides one-on-one "hands on" guidance and support to clients who would most benefit from access to and full engagement in primary HIV/AIDS medical care and HIV prevention services, as well as Mental Health and Case Management services, and LGBTQ+ services as needed.

The Patient Educator will be responsible for task related to managing client cases including services such as: health education sessions, linkage to care services, adherence and retention services, follow-ups and referrals/linkage, and supporting providers in obtaining Prior Authorizations.

The Patient Educator will also provide outreach services to support patient engagement, adherence and retention in care and treatments (phone calls, letters, face to face, text messaging, etc.)

The Patient Educator will also provide support for Social Media content creation and posting such as creation of content, scheduling of posts, engagement of users and referrals as needed.


Knowledge Required

  • Spanish Speaking- required
  • Working knowledge of HIV/AIDS and HIV prevention education, treatment, and partner services - required
  • Experience working with LBGTQ individuals - required
  • Proficiency in AIRS database preferred
  • Experience working with at risk individuals who may be partners of clients served- required
  • Experience working with individuals living with HIV - required
  • Experience with one/or more of the following: Instagram, Facebook, SnapChat, Twitter
  • Experience using an EMR (EPIC, etc.)
  • HIV counseling and testing
  • Proficient in Microsoft Office
  • Ability to use everyday office equipment



High School Diploma Required/Bachelor's Degree preferred
