The Community Health System Lab has established a new Community Health Worker Center of Excellence at Montefiore Health System (MHS) that harnesses both the science of implementation and expertise of communities to reimagine how community health can be delivered. The aim of the Community Health Worker Center is to establish a sustainable and scalable framework to formalize and promote the education and deployment of Community Health Workers (CHWs) to address the social determinants of health. We are looking for a motivated Program Manager to operationalize the Community Health Worker Center and direct a growing team of CHWs. Join us on this dynamic new team to improve health outcomes.


The Program Manager will work closely with Community Health System Lab (CHSL) staff and senior management team, and report directly to the Director of the CHW Center to effectively develop, maintain, and grow the CHW Center.  This position will be engaged in all aspects, based on skill level, of implementation and ongoing management of CHW Center programs/initiatives.  


Reponsibilities include:

  • Contribute to program design for the CHW Center
  • Research evidence and/or practice-based strategies for implementing CHW programs and addressing the social determinants of health
  • Serve as program content and technical advisor for the CHW Center
  • Explore and integrate innovative approaches to engage community members
  • Support CHSL strategic planning, and organizational development and improvement
  • Develop and utilize detailed work plans and timelines to guide achievement of grant/contract/partnership commitments and goals
  • Monitor program outcomes, data collection, and implement quality improvement strategies to strengthen program implementation
  • Develop and leverage relationships with internal and external stakeholders
  • Ensure clear communication of program goals, results and lessons learned
  • Work closely with program evaluation and research arm of CHSL to inform program evaluation, analyze, document and share findings.



  • Provide overall management of CHW Center operations and staff
  • Manage CHW supervisors and provide guidance and support for overall implementation strategies that align with strategic plans.
  • Ensure grant deliverables and contract commitments are met and the program remains compliant with funding guidelines, including timely and complete technical reports.
  • Ensure appropriate use of funds and timely and complete financial reports.
  • Lead integration of CHW workforce at clinical sites and develop operational processes for standardization of work
  • Develop comprehensive training plan for CHW Supervisors and CHW team
  • Coordinate continuous quality improvement efforts
  • Assist in development of funding proposals, progress reports, program publications and other content and material to share successes and lessons learned.
  • Conduct and manage annual performance reviews for RH grant staff members



Education/Skills required:


  • Master’s degree in a related field, plus 5 years of related experience is required.  
  • Management and administrative experience with teams (>5 people) is required.
  • Experience in the public health field; familiarity with community health workers preferred.  
  • Experience managing grant funds and/or contracts
  • Report writing and preparation.
  • Strong communication skills.
  • Bilingual in Spanish or French preferred.
