SOCIAL WORKER MSW

BRONX, NY, US


As a member of the renal and kidney transplant service, this social worker helps patients and families cope with kidney disease.  The social worker is part of an interdisciplinary team who work together to help patients and families adjust to the changes that come with dialysis or kidney transplant.  The social worker ensures compliance with CMS guidelines for dialysis which includes home visits for patients on peritoneal dialysis.  The renal social worker works closely with the transplant social worker to complete the required psychosocial evaluation to determine a patient’s eligibility for transplant.  The social worker then works with the patient and family on any barriers to transplant.  As these patients and families face chronic and life threatening illness, the social worker is also present to assess and support the ongoing emotional and concrete needs.  This position functions across care settings by meeting patients and families in the ambulatory renal clinic, outpatient dialysis unit,  and serving as the lead discharge planner from the inpatient arena. 

 

Experience with pediatric and adolescent patients required

Previous experience with pediatric transplant patients, and pediatric patients with life threatening illness preferred.

NYS LMSW or LCSW licensure required
