Will work part of an interdisciplinary team at the Einstein campus. Inpatient Med/Surg units and will provide a range of direct and indirect services for patient's and families including but not limited to discharge planning and the identification of psychosocial problems and their solutions. Will collaborate with patient. families and team members to ensure safe and timely discharges and treatment plans.

This position is an alternate weekend, 9a-5p, license social worker.

This role is critical to the hospital's throughout initatives to decrease LOS and increase timely arrangements.

Additionally, safe and effective discharge planning as carried out by social workers is a CMS, DOH and JCAHO regulatory requirement.

Social Workers see 70% of all admissions for discharge planning and psychosocial services.




Will work alternate weekends (every other Saturday and Sunday).

LMSW or LCSW and NYS licensure required.

If employee has a limited permit, they must have an executed Memorandum of understanding with a licensed Social Worker for the purposes of supervision and must obtain LMSW within 6 months of employment. 
Education Level required:  Master’s Degree in Social Work
