This position is s a resource to referring providers, dialysis units, and patients in the Hudson Valley Region and the Bronx. Accountable for developing and maintaining relationships with referring providers and Dialysis Centers in order to bring patients in for evaluation for transplantation. The additional services this individual with is out-reach related events like patient education events, organized walks for fundraising, CME sessions and Practice dinners.
The specific job duties for the position consist of enhancing communication between the community physicians and transplant specialists, Dialysis Centers, acting as a key resource of information , monitoring needs and responding to concerns of community physicians, and facilitating patient intake into the transplant pathway. In addition, creating and maintaining referral sources from outside of the local community is a key part of the job. Another responsibility of the outreach social worker is to increase the profile and exposure of the the transplant team in the tri-state area through grand round lectures, dinner events, CME events, etc. The individual will maintain a log of their activities and report their activities on a regular basis to program leadership.
This position requires a knowledge of the transplant evaluation process and the unique features of the transplant center at Montefiore. This knowledge will be part of the individual’s orientation and education if he/she does not have a transplant background.
Master's degree in Social Work required
Some healthcare experience necessary. The key is someone who is self-motivated, has the ability to communicate and build relationships with people, great interpersonal skills and have proficiency in .
3 Years work experience preferred.
