TRANSPLANT COORDINATOR PRACTITIONER

BRONX, NY, US


The Clinical Transplant Coordinator/Practitioner, in collaboration with physicians and other health care providers, will be responsible for the planning, evaluation, coordination, implementation and facilitation of all facets of care for patients  and families undergoing the process of organ transplantation.  Demonstrates advanced and diverse clinical knowledge and skills in the capacity of clinician, educator, researcher, collaborator and patient advocate to provide both direct and indirect patient care in a semi-autonomous, self-directed role in accordance with professional practice standards and clinical expertise.  Coordinators who are Physician Assistants or Nurse Practitioners will, in addition, be responsible to their supervising physicians for clinical matters as required by state and hospital regulations. Required licenses and registrations:

 

  • Current licensure and registration (or valid permit) as a registered professional nurse with the New York State OR
  • Current certification and registration as a nurse practitioner with the New York State OR
  • Current licensure and registration as a physician assistant with the New York State
