Pediatric Heart Transplant Coordinator

The Montefiore Medical Center Heart Transplantation and Ventricular Assist Device Programs are busy, growing, nationally recognized programs with outcomes that are among the best in the nation. In collaboration with the Medical and Surgical Directors, Cardiologists and other health care providers, will be responsible for the planning, evaluation, coordination, implementation and facilitation of all facets of care for pediatric patients and their families undergoing the process of cardiac transplantation. The candidate will demonstrate advanced and diverse clinical knowledge and skills in the area of cardiac care and will assume primary responsibility for all transplant candidate, recipients and family education, utilizing age specific skills and competencies, based on patient learning needs, chronological maturation, emotional status and cognitive ability. He/she will coordinate the diverse outpatient clinical management of transplant recipients during all phases of the transplant process, participate in the QAPI process, work closely with the multidisciplinary team to achieve goals. He/she will maintain clinical records and database systems utilized to capture and track information for the cardiac transplant patient.  This includes the timely submission of transplant tracking forms and the adherence to UNOS requirements for adding and removing patients from the waitlist. Appropriate implementation of established clinical protocols and the ability to seek guidance from the collaborating physician when complex medical issues arise.

Participate in an “on call” rotation for organ procurement with one other Pediatric Coordinator.   



Nurse Practitioner or Physician Assistant

Licensed in the State of New York

3+ years’ experience in cardiac patient care preferred

Experience in transplant patient care role preferred

Proficient in computer related skills

Effective English communication skills, both written and oral.
