CARE MANAGEMENT ANALYST CMO

YONKERS, NY, US


Licensure or certification in a health or human services discipline (e.g. LPN, LCSW, LMSW,) or in lieu of licensure/certification a degree in social work, or another health or human services field with appropriate training and experience in case management as determined by the hiring manager. Working under the supervision of a licensed certified clinical professional, the Care Management Analyst (CMA) collects and organizes information from multiple sources including members, care givers, physicians, clinical information systems and electronic health records. The CMA uses this information to assist with the development of an individual plan of care for each member. This may incorporate investigating which health care services will be beneficial for the member and/or directs this organizational and/or departmental protocols, policies and procedures. The CMA assists with managing the above activities for a case load of members or may manage specific processes and tasks related to support for case management activities. The CMA interfaces with and acts as a resource to team members, other MMC and CMO departments, members/caregivers and physicians/care providers
