The Transformation Manager works collaboratively with the Medical Director and the Director of the Quality and PI division to design, execute, and drive organizational change and performance improvement at the Montefiore Medical Group (MMG).

This position is responsible for ensuring ongoing compliance with NYS PCMH standards and attaining of the highest level of recognition, and will support Division’s Data Analytics Team in monitoring and reporting MMG compliance with other quality based programs, including the Health First Quality Incentive Program and the ACO/Next Generation Model.

The Manager will apply project management tools and principles in assisting the Director for Quality and PI with oversight of the Transformation Team and coordinating quality and performance improvement support for 21 medical group sites.  This individual will work closely with Transformation Coaches to facilitate skills transfer and problem solving capability development at various levels throughout the Montefiore Medical Group.

Bachelor's Degree Required, Master's Degree Preferred       

    • 3 - 5 years of project management experience, preferably in healthcare related fields, PMP certification- not required

   • 3 - 5 years of experience applying and coaching performance improvement principles, tools and concepts

   • 2 - 3 years of supervisory experience

   • Applied experience with Lean, Six Sigma or other Performance Improvement methodologies and tools
