BRONX HEALTH COLLECTIVE MEDICAL DIRECTOR

A mission driven, innovative federally qualified health center (FQHC) located in the South Bronx is seeking a full-time Family, Internal Medicine or Medicine-Pediatrics physician to join our team and provide leadership for adult and family medicine providers working at our FQHCs and at mobile and shelter sites for families experience homelessness.  Responsibilities will include oversight of clinical services including quality, productivity, and optimization of care models to respond to patient and community needs.  The Bronx Health Collective (BHC) is a program of Montefiore Medical Center, affiliated with Albert Einstein College of Medicine, and committed to providing high-quality, comprehensive, and compassionate health care.

 

Our beautifully designed and technologically sophisticated community health centers provide an enhanced medical home with comprehensive primary care in Pediatrics, Reproductive Health, Adult Medicine, and HIV/AIDS care to underserved populations. Behavioral Health and Nutrition services are co-located and integrated into primary care. Our community health centers have received and maintain NCQA’s Patient Centered Medical Home recognition with Behavioral Health Integration Distinction.

 

Special health initiatives include Child and Family Resiliency, Prenatal Group and Well-Baby Group, Diabetes, Asthma, Obesity Prevention, and Terra Firma, a medical-legal-mental health partnership clinic for unaccompanied immigrant children and families seeking humanitarian protection. Additionally, workgroups for staff to address issues of Trauma Informed Care, Antiracism and Equity, and LGBTQIA healthcare. Case managers, patient navigators, data analysts and public health specialists are an integral part of the health care team.

 

Qualifications:

  • Current unrestricted NYS medical license and unrestricted DEA certificate, Infection Control and Basic Life Support certifications.
  • Board Certified/Eligible in Family Medicine, Internal Medicine, or Medicine-Pediatrics (MD or DO).
  • Experience and/or interest in a community-based setting.
  • Excellent verbal and written communication skills.
  • Comfortable with electronic medical records.
  • Bilingual (English/Spanish) is a plus.
  • Minimum 5 years supervisory experience.
