CLINICAL PHARMACY MANAGER

BRONX, NY, US


As part of a dedicated multidisciplinary team, the cardiology transitional care clinical pharmacy manager provides inpatient and outpatient clinical services and works in collaboration to achieve expected program outcomes of reduced readmissions and emergency department utilization in patients with heart failure and acute myocardial infarction.

 

Responsibilities 

  • Assesses health care needs and solves medication-related problems
  • Collaborates with inpatient and ambulatory teams to ensure effective care coordination
  • Follows an assigned cohort of heart failure and AMI patients for post discharge transitional care management visits via telemedicine and/or remote patient monitoring
  • Optimizes evidence-based pharmacotherapy through collaborative drug therapy management (CDTM) agreement
  • Serves as the patient/caregiver resource for transitional care information and follow-up care coordination during the transition period
  • Serves as liaison to post acute clinical partners (home care, skilled nursing facilities)
  • Engages in performance/quality improvement activities and promotes transitional care excellence throughout system

 

 

Qualifications 

  • Doctor of Pharmacy degree 
  • Advanced training (e.g., completion of a PGY-2 residency in cardiology, ambulatory care, or other relevant specialties) or completion of a PGY-1 residency with current board certification and 3 years of relevant experience
  • NYS license with eligibility to be credentialed as an allied health professional
