HOME HEALTH NURSE

BRONX, NY, US


As a Home Health Registered Nurse you will:

  • Act as liaison between patient, family, and physician
  • Provide updates on the patient condition to family and physician
  • Ensure all in-home medical equipment are in working order
  • Document all details of patient’s condition, progression and regression activity
  • Oversee required physical therapy or rehabilitation exercises
  • Contact emergency services when patients need to be taken to the hospital
  • Possess knowledge of Home Care reimbursement process
  • Possess knowledge of OASIS
  • Have reliable transportation as you go to/from home visits
  • Can walk up/down six (6) flights of stairs as needed

 

Requirements:

  • Minimum of one (1) year hospital Med Surg experience
  • One (1) year of Home Health experience | Certified Home Care (CHHA) experience (Preferred)
  • NYS RN License & Registration in good standing
  • Bachelor's degree
  • Infection Control certificate
  • Current CPR certification
  • Valid Driver License, Registration and Automobile Insurance
  • Knowledge of a second language a plus
