Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek. Probationary period will not apply to shift changes.
Oversees a caseload of patients and acts as the primary liaison between the patient, the Gastroenterologist and the Primary Care Physician. The Patient Navigator is responsible for building a relationship with each screening program which includes colorectal cancer, liver transplant, abdominal transplant, cardiac transplant, and hepatology patients, while addressing barriers, developing relationships, tracking outcomes, and increasing prevention. During this process, they educate the patient with the importance of the procedure, while prepping the patients. The Patient Navigator links referrals to office visit and confirms admissions via EPIC. Prioritizing FIT Test patients appointments and arranging appointment time with physician to add diagnostic colonoscopy.
The Patient Navigator confirms candidacy, schedules, and increases compliance with reminder calls. Provide basic program and prep education, and documents into the Electronic Medical Records. This includes phone calls.
Education Preferred Level- Bachelor's Degree
Education Required Level- Bachelor's Degree
Preferred Bachelor of Science in Public Health, Biology, Psychology, Education or an equivalent degree OR Have at least four years’ experience in a community health role.
Must possess knowledge of community resources and hospital processes, structure, and function. Bilingual in Spanish
Facility with personal computers and data base software preferred. Work effectively in a team environment.
