Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek. Probationary period will not apply to shift changes.
This position is responsible for organizing and coordinating the comprehensive social service case management of high risk BHC patients and families, as well as for developing and coordinating substance abuse treatment and domestic violence problems. Specifically, the Social Worker will provide supportive counseling, crisis intervention services, assistance with concrete services/entitlements, assist with linking up new families with our existing medical sites and will also assist with securing appropriate referrals/linkages and other related duties as required.
This position will be responsible for conducting psychosocial assessment and clinical intervention/treatment of individuals and families enrolled in the BHC, and for developing, coordinating and providing case management/social work services for individuals and families .
Additionally, the position entails responding to diverse and unpredictable patient and family needs, provider demands and contract compliance in a professional and knowledgeable manner. Work on special initiatives to meet grant deliverables.
Educational requirements include NYS M.S.W. /L.C.S.W, license and registration with a minimum of one year of direct case management/social services experience in a community-based setting caring for underserved populations. Experience with domestic violence case management issues preferred. Spanish Speaking required
