Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek. Our Social Services Department is currently seeking a highly skilled Social Worker to provide exceptional care to patients on a per-diem basis, with a regular schedule every Sunday 2pm-10pm


Position Summary:
The Social Worker will provide a range of direct and indirect services for patients and families, including the identification and resolution of psychosocial issues. This role involves collaboration with patients, families, and interdisciplinary teams to ensure safe and timely discharge and treatment planning. The Social Worker will uphold the mission of Montefiore and the Social Services Department, as well as the Code of Ethics of the National Association of Social Workers.

Key Responsibilities:

•    Screen patients for social service needs based on diagnosis and social data.
•    Conduct psychosocial assessments through interviews and interdisciplinary collaboration.
•    Develop and implement discharge/treatment plans in a timely, collaborative manner.
•    Function independently while utilizing supervision for complex discharge planning.
•    Maintain professional conduct, confidentiality, and participate in interdisciplinary conferences.
•    Demonstrate leadership in team building, conflict resolution, and psychosocial education.
•    Participate in departmental initiatives including quality improvement and continuing education.
•    Apply knowledge of legal and ethical issues related to end-of-life care and patient rights.
•    Identify and intervene appropriately in cases of abuse, neglect, and violence.

Qualifications:

•    Master’s Degree in Social Work from a NYS-registered or equivalent program.
•    LMSW or LCSW licensure preferred; otherwise, must have a Memorandum of Understanding with a licensed Social Worker for supervision.
•    Strong understanding of the impact of medical, social, and environmental factors on patient care.
