CARE TRANSITIONS CLINIC COORDINATOR

The Care Transitions Clinical Coordinator (CTCC) oversees all transitional care activities for a designated case load of patients, to ensure a timely transition along the care continuum. The CTCC is an active participant of the Interdisciplinary Team who contributes input into the management planning for all assigned patients. The CTCC identifies and escalates complex discharge concerns to the appropriate party. The CTCC oversees the discharge appeal process and manages all SNF returns. The CTCC demonstrates core and unit specific competencies based on the patient population served. Candidate must demonstrate proficiency in use of computer, including data entry, word processing and email. The CTCC is familiar with current health care delivery systems and has an understanding of post-acute care options. The CTCC has understanding of chronic care management, as well as palliative and hospice care.
Requirements for CTCC position NY State RN License, BSN degree at a minimum with 5 years of clinical nursing experience.
The CTCC must be PRI certified with proof of certification.
Board certification in Case Management is preferred
Daily work schedule 8:30am- 5pm - 5 day work week, weekends are included. Rotating Schedule.
