Overview:

The Home Care Liaison facilities referrals to home care and/or hospice by gathering and interpreting medical and financial data, meeting with patients and families, and working with case managers to ensure optimal transition from hospital or other facilities to home.

 

Job Responsibilities:

 

  • Makes initial contact for clinical, social, psychological and financial assessment of home care and/or hospice services
  • Provide accurate and thorough documentation either via phone, fax, or computer to agency for additional data entry
  • Assists with patient's insurance to determine home health and/or hospice coverage
  • Interprets home care and hospice policies and acts as a resource for case managers, physicians, hospital and community for effective public relations and marketing of services

 

 

Education & Experience Qualifications:

  • BSN ( Preferred)
  • Licensed & Registered RN nurse
