CLINICAL MANAGER

The Manager will track and monitor all psychiatry discharges for 77 beds across MMC and Mt Vernon. The Coordinator will be responsible for the timely communication and coordination across Moses, Wakefield, Weiler and Mt Vernon Psychiatry teams across the inpatient and ED settings.  The goal is to facilitate a seamless transition for high acuity psychiatry patients discharged from the ED, medical, and surgical settings, ensuring a smooth hand-off and placement to utilize full system acute psychiatry capacity.  Additionally, the coordinator is expected to offer case management support for highly complex cases in our inpatient psychiatry and medical settings. 

 

Timely communication and coordination across Moses, Wakefield, Weiler and Mt Vernon Psychiatry teams across the inpatient and ED settings.

 

Evaluating and assessing psychiatric admission needs from ED, medical and surgical settings

 

Ensuring timely communication and placement of patients post-discharge

 

Provide case management support for complex cases 

 

Attend complex care meetings at each campus

 

Coordinate communication across the entire system concerning the availability of psychiatric beds, patients awaiting bed placement, and those awaiting discharge.

 

Leading cross-campus huddles multiple times per week to discuss key data points and patient needs across campuses and providing direction and decision-making on key placement issues 

 

 

Requirements

 

Education BSN or LCSW

NYS license and Registration

Five years experience in a clinical setting

Strong leadership skills required

Knowledge of EPIC, Microsoft, and Outlook
