CLINICAL MANAGER

The clinical nurse manager is a member of the Health Home leadership team who will provide clinical, administrative and educational consultation aligned with Health Home Policies and Procedures, Best Practices, Quality Initiatives and Revenue Targets. The Clinical Manager assists in the development of policies, procedures, work flows and program monitoring to resolve barriers to successful implementation of the Health Home Care Management Program. Position supports staff development and training. The Clinical Nurse Manager will supervise care managers as needed.

 

This position requires

• Masters degree in nursing or a health-related field (i.e. DPT, physician assistant)

• Current active license/registration in New York State as applicable and additional state licenses/registration (NJ, CT, etc) as required by Program.

• Medical Management experience in a managed health care setting preferred.

• 5 years of clinical practice, preferably including at least 2 years in an outpatient setting .

• Minimum 2 years of supervisory experience

• Familiarity with computers and computerized case management and health information technology systems.

• Fluency in utilizing excel spreadsheets and technology.

• Knowledge of: - Medicare & Medicaid benefits and guidelines - case management concepts

• quality improvement techniques
