Job Title: CLINICAL PHARMACY MANAGER

Description 

 

As part of a dedicated multidisciplinary team, the transitional care clinical pharmacy manager provides inpatient and outpatient clinical services and works in collaboration to achieve expected program outcomes of reduced readmissions and emergency department utilization in patients with complex and chronic conditions. This position is responsible for effectively enhancing the drug therapy outcomes of patients at the Medical Center specifically related to transitional care.  This is accomplished through continual assessment of the appropriateness, safety, efficacy and outcomes of pharmacotherapy medication through direct patient care, education to health system personnel and the provision of pharmacotherapy-related outcomes research and performance improvement initiatives.

 

Responsibilities 

  • Assesses health care needs and solves medication-related problems
  • Collaborates with inpatient and ambulatory teams to ensure effective care coordination
  • Follows an assigned cohort of patients for post discharge transitional care management visits via telemedicine
  • Optimizes evidence-based pharmacotherapy through collaborative drug therapy management
  • Serves as the patient resource for transitional care information and follow-up care coordination during the transition period
  • Serves as liaison to post acute clinical partners (home care, skilled nursing facilities)
  • Engages in performance/quality improvement activities and promotes transitional care excellence throughout system

 

Qualifications 

  • Doctor of Pharmacy degree 
  • Advanced training (e.g., completion of a PGY-2 residency in cardiology, ambulatory care, or other relevant specialties) or completion of a PGY-1 residency with current board certification 
  • NYS license with eligibility to be credentialed as an allied health professional  
  • Working knowledge of Microsoft Office (Word, PowerPoint, Excel), Data Mining software (e.g. Web Intelligence, Clinical Looking Glass), Epic Electronic Heath Record system.  If necessary, remedial work will be done upon hire.
