FIELD MANAGER

BRONX, NY, US


This position will supervise work of assigned Navigator staff, who will be responsible for providing Navigation services to all WeCARE clients.  The Supervisor's role is to lead Navigation staff, ensure they comply with all NYC HRA contractual requirements, and to cover for the Assessment Supervisor in his/her absence.

Assist with clinical assessment/reassessment/coordination/communication with other Qualified Health Professionals. Skills navigating clients in clinical assessments includes:  
Medical/health diagnoses and impact on individual's functional capacity;  
Behavioral health diagnoses and impact on individual's functional capacity; 
Requirements to qualify for Federal Disability Benefits. 

 

Bachelor's Degree required, Master's preferred. LCSW, LMSW, CRC or HMHC required.

3 years exp Health Care Administration,   Knowledge of medical and/or behavioral health diagnoses.
