Home Care Senior Physical Therapist:

Responsibilities:
• Assesses, plans, coordinates, implements and evaluates plans of care for clients in their home, in accord with professional standards, regulatory guidelines and agency policies
• May be required to train or assist in training other staff
• Instructs and evaluates ancillary personnel on their role in the plan of care
Requirements:
• Graduate of a program in Physical Therapy approved by the American Physical Therapy Association.
• Current New York State Licensure
• Two (2) years at senior level in field or acute medical hospital
• Two (2) years supervised clinical experience in a medical setting preferred, or equivalent combination of education and experience
• Must have a valid driver’s license and ability to provide own transporting during working hours for the purpose of making home visits
