LIVER CLINICAL TRANSPLANT COORDINATOR

The Liver Clinical Transplant Coordinator (RN) works collaboratively within the multidisciplinary healthcare team and is responsible for the evaluation and wait list management of the liver transplant patient. Provide professional support, planning, coordinating, and facilitating patient care across the continuum of care, ensuring compliance with orders and established policies and procedure. Demonstrates advanced and diverse clinical knowledge and skills in the capacity of clinician, educator, collaborator, and patient advocate to provide semi-autonomous, self-directed role in accordance with professional practice standards and clinical expertise.  Responsibilities include coordinating patient communication, patient education, transplant evaluation and management of the liver transplant patient once listed for a transplant.  Incorporate quality assessment, performance improvement, clinical research, and teaching to maximize and improve patient care activities.

 

Location: Outpatient

 

Essential Functions:

  • Conducts education and evaluation for potential liver transplant recipients
  • Expedites appropriate care and treatment, using critical reasoning and astute clinical judgment especially given the patient may present with complex problems or potentially life-threatening conditions
  • Coordinates care with physicians and health care team
  • Present patients at Liver Selection Committee
  • List and manage patients while waiting for a liver transplant on the wait list
  • Maintain MELD scores and submit exceptions for waitlisted patients

 

Requirements

BSN, required

Master’s degree, preferred

Current licensure and registration (or valid permit) as a Registered Nurse with the New York State

A minimum of 4 years of clinical experience in a highly specialized complex care program.

Transplant experience, preferred

Spanish speaking, required

Effective communication skills, both written and oral

Ability to use data entry and retrieval system
