The Licensed Practical Nurse performs nursing services within the framework of direct patient care, case finding, health counseling and supportive restorative care under the supervision of a Registered Professional Nurse. Nursing care is provided within the standards of nursing practice as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and nursing’s Social Policy statement.

 

The LPN participates in performance improvement /research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills. The LPN demonstrates core and unit specific competencies based on the patient population served. 
The LPN supports the mission, vision, critical success factors and goals of MMC and standards of nursing.

 

RESPONSIBILITIES:
•    Provide direct care for a population specific patient assignment reflective of current and status of patient.
•    Demonstrate medication administration as per regulatory requirements and MMC policy and procedure
•    Provide patient / family teaching to meet learning needs – how, why, and when medication is taken and provide appropriate medication storage information.
•    Set priorities, problem-solve, and use proper judgment in difficult situations and be flexible in non-emergent situations
•    Provide and maintain safe environment for patients/families through preventative and interventional measures
•    Document patient care per policies and procedures
•    Participate, promote, and practice continuous quality improvement. Collect and document of data relevant to planning / evaluation of patient care
•    Responsible and accountable for own practice
•    Demonstrate body mechanics as it relates to lifting / transfer / procedural assistance and knowledge and skills necessary to provide care appropriate to patient population served
•    Use Blood pressure, temperature, breathalyzer, pulse oximeter equipment, electronic clinical information system, and perform phlebotomy

 

QUALIFICATIONS:
•    High School Diploma / GED, Associate’s Degree preferred
•    New York State License for Practical Nurse (LPN) in New York State, Basic Life Support Certification are required
•    Effective English communication skills, written, oral and interpersonal
•    Ability to use basic data entry and function in an Electronic Health Record
•    New York State Mandatory Infection Control Certification required
•    Working experience performing phlebotomy, using EKG machine, weight scale, glucometer and additional medical equipment
•    Experience using automated medication dispensing system and literacy, medication administration, and scientific instruments

 

The hours for this position are 7:30a-3:30p Monday - Friday and 6:30am -2:30pm on alternating Saturdays. This is a float position and requires flexibility to work at all Wellness Center locations and various shifts as needed. 

 

This is a 100% grant funded position.
