Licensed Practical Nurse (LPN) - Diabetes remote patient monitoring program

 

 

We are seeking a detail-oriented Licensed Practical Nurse (LPN) to join our Remote patient monitoring program. The LPN will play a key role in our multidisciplinary team (consisting of physician, nurse practitioner, endocrinology fellow, and LPN) patient care, assisting with continuous glucose monitoring system (CGM) placements, patient education, and ensuring seamless access to the clinic portal. The LPN would have a day dedicated to the RPM program, in which they would be doing a mix of video/telephone calls and in-person visits to assist with the following responsibilities of the clinic:

 

This is a grant funded position.

 

Responsibilities - include but not limited to 

Assist with CGM placements and provide thorough patient training on device usage and troubleshooting.  - For patients with new devices, will need in person visits for placement of CGM and set up of phone with our clinical portal.

Educate patients on glucose trends and downloading CGM data for future review.  

Download and upload CGM data into the patient's electronic health record (EHR) for upcoming visits.

Facilitate patient access to the clinic portal and troubleshoot connection issues.  

Collect medication recall and dietary recall information to update medical records.  

Perform routine clinical tasks such as updating charts, managing prescription renewals, ensuring patients have their prescriptions (and if they do not, what issues they had ie prior authorization issue, drug shortage, etc)

Assist with preparation for patient consultations and follow-up appointments.

Provide education on diabetes management, including medication adherence, dietary modifications, and lifestyle advice.  

Help patients interpret their CGM data and apply insights to their treatment plans.

Participation in team meetings reviewing the flow of RPM clinic, providing feedback for quality improvement in how we can better work as a clinic to get our patients to their glycemic goals and improving our workflow.

 

REQUIREMENTS:

High School Diploma or equivalent.

LPN Training program and NYS LPN License and current registration to practice as LPN.

Basic Life Support

At least one year (1) outpatient experience.   Endocrine/Diabetes related experience strongly preferred.

Effective English communication skills, written, oral and interpersonal.

Ability to use basic data entry and retrieval system.

Additional education required by specialty areas/patient care locations.

Ability to use automated clinical information system, personal computer, telefax, photocopier, telephone, centrifuge, EKG Machine.erilization process and/or Autoclave Machine where applicable.
