Job Title:  Family Nurse Practitioner FNP / ANP

 

The Nurse Practitioner will assess and provide care to a selected group of patients in collaboration with physicians. The NP will be responsible for carrying out the plan of care for his/her patient panel. The NP contributes to the actualization of the philosophy and objectives of the specific Division/ Department. He/she practices within the scope of practice in care delivery. He/she participates in performance improvement and research activities and maintains professional practice standards and clinical expertise.

 

This position will work at the Montefiore Bronx Health Collective (BHC), a mission driven, innovative community health center located in the South Bronx. BHC is Federally Qualified Health Center and all programs have received NCQA’s Patient Centered Medical Home Level 3 recognition. BHC’s community health center and homeless healthcare program provide comprehensive primary care in Pediatrics, Reproductive health, and Adult/Family Medicine. 

 

BHC provides an enhanced medical home with comprehensive primary care in Pediatrics, Reproductive Health, Adult Medicine, and HIV/AIDS care to underserved populations. Mental health and nutrition services are co-located and integrated into primary care. Special health initiatives include Child and Family Resiliency, Prenatal Group and Well-Baby Group, Diabetes, Asthma, Obesity Prevention, and Terra Firma, a medical-legal-mental health partnership clinic for unaccompanied immigrant children and families seeking humanitarian protection.

 

BHC also provides reimbursement for renewal of NYS license and DEA registrations, discretionary account and 3 CME days and participates in the Loan Repayment Program via HRSA.

 

EDUCATION, TRAINING & EXPERIENCE:

A. New York State License for a Registered Professional Nurse and current registration to practice in New York State as an RN/ NP.

B. Current New York State Nurse Practitioner specialty certification and registration.

C. Masters or post Masters Certificate in specialty as a nurse practitioner preferred; or a combination of education and experience that is acceptable to N.Y. State Education Department and the Director of Nursing and/or Chairman of the Department, or Division Director.

D. National certification in nurse practitioner specialty (i.e.: ANCC, AACP or NAPNAP) preferred.

E. Additional education as required by specialty/patient care location.

F. Effective English communication skills, both written and oral     One (1) year of experience as an FNP.  Primary care experience is preferred.

G. Ability to use data entry and retrieval system.
