NURSE PRACTITIONER

DESCRIPTION:
The Hemophilia Treatment Center (HTC) is hiring a Nurse Practitioner (NP) to assist in providing clinical services to both our adult patients with hemophilia, von Willebrand disease and other rare bleeding and thrombotic disorders. The NP will work closely with adult hematologists who direct our HTC. The NP will report on all clinical and programmatic matters to the Medical Directors of the adult and pediatric practices of the Hemostasis and Thrombosis Treatment Center. The HTC NP position is a unique opportunity that combines working closely with patients and families affected by coagulation.
SUMMARY OF RESPONSIBILITIES:
- Direct and provide clinical care for adult patients of the HTC. Independently assess patients and present management plans.
- Provide direct and supportive services for the attending MDs, including patient education, prescriptions, and the development of treatment plans. Document patient visit in EMR within 24 hours.
- Triage telephone calls, interview patients, and provide self-care instructions.
- Communicate with other medical / surgical / dental services and act as a liaison to ensure continuity of care for the patients.
- Direct and coordinate resources for the delivery of care to the patients and families of the HTC
- Prescribe factor and therapeutic product to patients; organize product delivery from the New York Blood Center and other organizations. Organizes home and/or hospital infusions. Maintains detailed and organized records up to date for each patient.
- Follow HTC patients in the hospital when/if they are admitted.
- Collaborate with Research Coordinator regarding projects of the American Thrombosis and Hemostasis Network (ATHN) to comply with the CDC and HRSA.
- Assist with patient eligibility determination for active and new research studies of ATHN, the region, and local investigators of our HTC.
- Support and participate in clinical research efforts of HTC and in all aspects of clinical research protocols, including IRB submissions, renewals, consenting.
EDUCATION AND REQUIREMENTS:
Valid New York State Nurse Practitioner License and Registration.
Experience and knowledge in providing care to Hematology patients, including patients with bleeding and clotting disorders, in both in-patient and out-patient settings as a Nurse Practitioner.
Excellent communication skills with children, adults and families is required.
Experience in clinical research is a plus.
Bilingual (English/Spanish) is preferred.
The position involves traveling between Einstein (East) and Moses (West) campuses and some professional travel to regional and national meetings.
