NURSE PRACTITIONER

The Nurse Practitioner functions to deliver direct patient care as well as manage administrative tasks that support primary care services at a MMG primary care site.  Nurse practitioner will function under the supervision of a physician.  This provider will be expected to provide clinical services both face to face, and telemedicine visits based on access and patient demand. This position is for a certified NP that has at least 2 years of post-training OB/GYN practice experience.

 

Education and Experience:

  • New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an RN
  • Current New York State Nurse Practitioner specialty certification and registration
  • Master’s or post Master’s Certificate in specialty as a nurse practitioner preferred or a combination of education and experience that is acceptable to NY State Education Department and the Director of Nursing and/or Chairman of the Department, or Division Director
  • National certification in nurse practitioner specialty (ANCC, AACP, or NAPNAP) preferred
  • Effective English communication skills, both written and oral
  • BLS certification

Specific Accountabilities:

    1. Primary Care Coverage
      1. Assigned to a specific MMG site to provide face to face clinical care, telemedicine and resolve clinical messages under the supervision of a MMG physician.
      2. Clinical care for patients as a primary care provider, including the assessment, evaluation, and management of patients including but not limited to: 
        1. Assess patient health by interviewing patients and reviewing patient histories.
        2. Performs physical examinations.
        3. Determines health status and abnormal conditions by administering or ordering diagnostic tests, such as x-rays, electrocardiograms, laboratory studies and interprets test results.
        4. Performs therapeutic procedures such as suture removal, administering injections and immunizations.
        5. Accurately documents patient care services by charting in electronic medical record in a timely manner.
        6. Provides continuity of care by developing and implementing patient management plans
        7. Instructs and counsels patients by explaining clinical assessments and therapeutic regimens.
      3. Clinical activities will include preventive, chronic disease, and acute care management.
      4. Participates in site’s clinical QI/ PI activities during days/ hours at the site
      5. Administrative responsibilities: includes Epic in basket coverage as expected for primary care providers including but not limited to:
        1. Forms
        2. Phone calls
        3. Results
        4. Medication refills
        5. MyChart messages
        6. Staff messages
        7. cc’d charts
      6. Clinical and administrative responsibilities will support patients and physicians of the entire practice.
