NURSE PRACTITIONER

SUMMARY OF POSITION:

 

The Division of Pediatric Endocrinology and Diabetes is recruiting a full-time pediatric or family nurse practitioner (NP) to deliver specialized care to our patients with diabetes. This candidate is expected to provide both medical care and education for all types of diabetes mellitus, including type 1, type 2, medication-induced, and genetic forms. Care will include assessment, diagnosis, management, and education, primarily in the out-patient setting, including telephone calls, but also if needed for assistance with diabetes education in the in-patient setting. The successful candidate will have their own independent outpatient sessions and be able to manage diabetes medications and technology.  The NP will perform history taking and physical exam, provide diabetes care education, interpret blood sugar patterns, and prescribe and adjust insulin, as well as medications for type 2 diabetes mellitus. The ideal candidate will already have certified diabetes care and education specialist (CDCES) certification and if not, will have personal or professional experience with the management of type 1 diabetes mellitus. Minimum requirements include a basic background knowledge of diabetes inherent to health professional academic training, including understanding hyperglycemia and hypoglycemia, POC blood glucose and ketone testing, and drawing up and injecting insulin. If the candidate does not have CDCES certification, they will be expected to sit for the exam within 2 years of hire. Once credentialled, the NP is expected to maintain CDCES certification.

 

INDEPENDENT JUDGEMENT:

 

Candidate must have the knowledge, skill, and ability to provide individualized assessment of learning needs and deliver content specific to these needs by:

  • Identifying the patient’s/guardians’ specific diabetes self-management goals
  • Implementing education and behavioral interventions directed toward helping the patient/guardian achieve identified self-management goals
  • Evaluating/monitoring the patient’s/guardians’ attainment of identified self-management goals and clinical outcomes
  • Ability to monitor patients' response to therapy and revise medication treatment plan as indicated.

 

EDUCATION AND REQUIREMENTS:

 

  • Valid New York State Nurse Practitioner License/Registration.
  • NP with CDCES  or personal or professional experience with the management of type 1 diabetes mellitus will be preferred.
  • NP with bilingual skills and prior experience in an outpatient setting will be preferred. 
  • Master's Degree in Nursing; Board certification as a Nurse Practitioner.
  • Relevant clinical nursing practice in the care of the patient population served; ability to apply nursing process toward achievement of specific outcomes in an organized fashion, which addresses patient/family safety and satisfaction.
  • Must have excellent interpersonal skills, effective communication skills, creative problem solving and excellent critical thinking and leadership. Demonstrated ability to facilitate group processes and work cohesively and collaboratively as member of the interdisciplinary team.
