OUTREACH SPECIALIST

As an integral member of the BCHN Community Health Education and Promotion Program (CHEPP) and BCHN Patient Care Teams, the Outreach Specialist will provide care coordination services and conduct outreach to improve patient care and community health outcomes with target populations and address their social determinants of health needs.
The Outreach Specialist will:

Convey the purpose and services of BCHN programs to the user population and the impact that programs or services would have

Coordinate and conduct health screenings, health fairs, individual and group educational workshops, and other health events

Conduct public education and outreach activities to raise awareness about health issues and facilitate access to BCHN health centers for primary care
- Motivate members to be active and engaged participants in their health

Will help community members develop health management goals and plans, and follow up to track progress and help meet their goal, providing care coordination services and serve as patient advocate

Assess for social determinants of health (SDH) needs and link and refer patients/clients to community resources and primary care services

Screen for health insurance eligibility and public benefits and assist with enrollment into a health plan or one or more of the public benefits that they are eligible for

Document every patient encounter, services provided and outcomes of referrals in EMR and BCHN reporting tools, submitting program reports in a timely manner

Have work experience with individuals with Substance Abuse Disorders
Knowledge, Skills & Abilities:
- Excellent interpersonal, communication and oral skills / Excellent public speaking to make presentations
- Proficiency in engaging the community and patients / Ability to communicate health education messages
- Knowledge of computer programs and proficiency in Microsoft Word, Excel and Power Point (Will also use photocopier and fax machine, as needed)
- Willingness and ability to work a flexible schedule to meet the needs of the community
- Second language a plus
Education:
High School Diploma/GED | Bachelor's Degree Preferred
Experience:
At least 1 year of experience in healthcare and/or care coordination and care management
Knowledge of care coordination and local community resources
