The Montefiore Patient Care Coordinator maintains standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing’s Social Policy Statement. The Patient Care Coordinator actualizes the vision, mission, values, and balanced scorecard performance measures for Montefiore Medical Center.  The Patient Care Coordinator participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills.  The PCC demonstrates core and unit specific competencies based on the patient population served. The Patient Care Coordinator (PCC) contributes to the actualization of the vision, of the philosophy and the balanced performance measures of the discipline.  He/she is committed to maintaining the standards of the practice of nursing and is responsible for the coordination, implementation, delivery, and evaluation of the care to the neonate, pediatric, adolescent, adult and/or geriatric patient.  The Patient Care Coordinator, in collaboration with and under the direction of the Administrative Nurse Manager participates in the coordination/implementation of fiscal, human, and other resources needed to provide patient care services for a designated unit, location or patient population on a designated unit. Monitors performance of nursing personnel in maintaining MMC standards of practice of nursing.  Coaches/ provides feedback to nursing personnel as needed.  Clinical title does not carry from unit to unit.

 

Responsibilities:

  • Demonstrate leadership skills in the coordination and delivery of patient care
  • Systematically assess the health care of patients/families and responses to care rendered
  • Demonstrate critical thinking skills to problem solve and initiate clinical decision making
  • Evaluate patient progress toward health goals and modifies critical path or plan of care accordingly
  • Develop, coordinate, and provide direct care for specific patient assignments
  • Provide safe environment for patients/families
  • Provide comprehensive direct patient care to assigned patients
  • Communicate effectively with patient/family and members of the health care team

 

Qualifications:

 

  • New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an RN required
  • Certification preferred in specialty or efforts to achieve certification
  • BSN degree required and 5 years clinical experience, two of which must be in the specialty or related service; or combination of education and experience acceptable to Clinical Director of Nursing or designee.
  • Demonstrates clinical leadership, problem solving skills, competency, and principles of Montefiore Excellence.
