The Patient Service Manager’s responsibilities are to plan, supervise, coordinate and

evaluate patient care services provided by the nursing staff employed directly by or through

contract with the CHHA/LTHHCP.  As the Patient Service Manager (PSM), you will supervise a staff of CHHA RNs (8-12) and provide clinical oversight.

Additionally, you will resolve operational issues that promote patient satisfaction.

 

Requirements:

  • License and current registration to practice as a Registered Professional Nurse in New York State
  • Baccalaureate Degree in Nursing from an approved program required. Masters Degree preferred
  • Two (2) years experience as a community health nurse with a Certified Home Health Agency or a Long Term Home Health Care Program
  • One (1) year experience in the field and one (1) year supervisory experience preferred, or an equivalent combination of education and experience
  • Must have valid driver’s license and ability to provide own transportation during working hours for the purpose of making home visits
