Job description - WeCARE Sr. Mental HealthSpecialist (QHP)

We are currently seeking a Senior Mental Health Specialist to join our esteemed institution at our Montefiore UBA-WeCARE Program. WeCARE provides a continuum of services to help HRA-referred cash assistance clients with medical and/or mental
health conditions that affect their employability to attain their maximum levels of selfsufficiency. Individuals referred to WeCARE receive medical and psychosocial assessments to identify possible clinical conditions and social barriers that may affect
their ability to participate in work-related activities. Based on the results of the assessments, a determination of an individual's functional capacity outcome (FCO) is made and a customized service plan by our team is developed to provide a range of
services tailored to meet a client's particular needs.

 

The WeCARE Senior Mental Health Specialist/Qualified Health Professional's (QHP's) role is to support the Supervisor of Assessment in the implementation of standard operation policies and procedures to ensure that UBA WeCARE complies
with all NYC HRA contractual requirements. Understanding of the criteria to meet listing requirements which appear in the SSA Blue Book of Impairments. Knowledge of common medical/mental health conditions as they relate to
documenting a relevant medical history. Knowledge of common medications for treatment of routine medical/mental health conditions in order to obtain an appropriate medical history.

 

Key Responsibilities:
1. Interview clients, review medical records/documentation, and confer with other WeCARE clinical staff to document clients' mental and physical conditions and to determine their suitability for participation in a specific program track including
Wellness, Federal Disability Services, Vocational Rehabilitation Services, or Employment without limitations.
2. Conduct the Social Security Administration Sequential Evaluation as part of the assessment process.
3. Suggest to the Review Physician referrals to psychiatrist, psychologist, or evaluating clinician as indicated.
4. Determine a Functional Capacity Outcome and write a supporting justification at the completion of the assessment.
5. Recognize when additional specialty assessments (Psychology, Medical) are needed to make a final FCO determination.
6. Additional duties as needed to ensure compliance with HRA and UBA's expectations.

 

Primary Job Duties:
1. Conduct comprehensive Clinical Assessment/Clinical Reassessment
2. Identify clients' need for Reasonable Accommodations (RA)
3. Determine Functional Capacity Outcome (FCO) and ensure Functional Capacity Outcome (FCO) determinations are explained to clients when completing the FCOfinalization
4. Track and document work activity
5. Maintain and properly store client provided documentation and documentation obtained from other electronic medical record systems (EPIC, BronxRHIO, Psyckes,etc.)
6. Receive direction and feedback from the Assistant Director of Clinical Assessment and Supervisor of Clinical Assessment regarding departmental policies, issues, and performance relative to project goals.
7. Work with Clinical Assessment leadership and Medical Team to learn clinical process and terminology of clinical assessments in the determination of Functional Capacity Outcome
8. Use clinical skills to successfully engage clients and diffuse tension so that they participate in the evaluation process.
9. Utilize workarounds for SEAMS issues as required.
10. Additional duties as needed to meet funder's (HRA) and Montefiore's expectations and performance goals

 

NYS professional license LMSW, LCSW or LMHC, Certified Rehabilitation Counselor (CRC), or other credentialed professionals as may be approved by HRA
