SOCIAL WORKER ASSISTANT (Grant Funded)

BRONX, NY, US


Under the direction of a licensed social worker, provide care coordination and comprehensive case management to health home clients with a range of chronic medical and mental health conditions and/or substance abuse.  Duties include care coordination, community outreach, referrals to community-based services, client engagement, advocacy, health promotion and field work. Assist clients with meeting their health care goals by gathering data and completing and updating basic assessments and plans of care per DOH regulations. Duties will also include processing forms and data entry in state required computer systems.

 

Must be organized and flexible to meet deadlines and prioritize competing priorities. Excellent supervision and educational opportunities available.

 

Candidates must have a bachelor’s degree in any of the following: child & family studies, community mental health, counseling, education, nursing with current NYS RN, occupational therapy, physical therapy, psychology, recreation, recreation therapy, rehabilitation, social work, sociology, or speech and hearing, plus two years of direct service with the medically complex, developmentally disabled, mental ill  or substance abuse populations. Those with Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses, or substance abuse or those with a Credentialed Alcoholism and Substance Abuse Counselor (CASAC) are also encouraged to apply. Bilingual preferred in Spanish or other languages.
