SOCIAL WORKER ASSISTANT

BRONX, NY, US


Under the direction of a licensed clinician, provide care coordination and comprehensive case management to health home clients with a range of chronic medical and mental health conditions and/or substance abuse. Duties include care coordination, community outreach, referrals to community-based services, client engagement, advocacy, health promotion and field work. Assist clients with meeting their health care goals by gathering data and completing and updating basic assessments and plans of care per DOH regulations. Duties will also include processing forms and data entry in state required computer systems. Must be organized and flexible to meet deadlines and prioritize competing priorities. Excellent supervision and educational opportunities available.

 

Candidates must have a bachelor’s degree in human services and/or a health related field, plus experience working with people with complex medical, psychiatric, or substance use conditions.  Computer literacy with online databases, Excel spreadsheets and electronic medical records required, Bilingual preferred in Spanish.
