This Licensed Social Worker (LMSW or LCSW) will have split responsibilities supporting both Montefiore Medical Group’s Behavioral Health Integration Program (BHIP) and the NYS Office of Mental Health’s (OMH) Project Teach program.   Two days a week the SW will provide therapeutic treatment to children, adolescents, and families referred to BHIP who are experiencing various mental health disorders including depression and anxiety. Three days each week will be spent supporting Project Teach as a liaison coordinator.  Project Teach is a multi-site OMH grant program designed to strengthen and support NY state’s pediatric primary care and Ob/Gyn providers in delivering mental health treatments to children and families.

BHIP Responsibilities: 
•    Utilize evidenced-based assessment tools to conduct clinical assessments and to track response to interventions provided
•    Incorporate evidenced-based treatment modalities.
•    Collaborate with an interdisciplinary team comprised of Primary Care Doctors, Psychiatrists, Nurses, and other internal and external professionals to improve the quality of patients’ lives
•    As needed, the Social Worker will also provide patients with assistance with Social Determinants of Health (concrete services) and make appropriate referrals to ensure patients’ safety at home and in the community.

Project Teach Responsibilities: 
•    Coverage of the Project Teach phone consultation line: 
o    Answering/triaging calls from providers across NYS
o    Providing behavioral health recommendations as appropriate
o    Coordinate connection of PCPs/OBGYNs to psychiatric consultant when indicated
•    Develop working relationships with community mental health resources to help facilitate patient linkage to services when needed
•    Curate list of mental health providers/groups/clinics in region to support linkage efforts within the region and throughout NYS
•    Attend weekly liaison coordinator meetings

Hours: Monday - Friday 8:30a – 5:00p
 
Education:
•    Master of Social Work
•    LMSW – Licensed Master Social Worker

Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek.
