Overview: 

The MAYS Clinic (Montefiore Adolescent and Youth Sexual Health Clinic) social worker will be responsible for collaborating with the patients care team to ensure that the patient is receiving appropriate support and coping mechanisms; including counseling and risk reduction. The social worker will provide a range of direct and indirect services, including but not limited to, the identification of psychosocial problems and their resolutions. This candidate will have the ability to make psychosocial assessments and act as a liaison between the patient and their care team. This associate will also be responsible for managing each patient’s medical support needs. The social worker will provide education to patients follow their progress. This associate provide risk reduction adherence counseling, mental health, substance use screenings/referrals.

 

Eligible candidate will be compensated as a 1.0 FTE. This position is 100% grant funded through the BFACC grant and AIDS Institute grant service contracts.

 

Requirements:

  • Masters in Social Work from a school of Social Work Accredited by the Council of Social Work Education
  • Five (5) years practice experience as MSW Social Worker
  • LMSW- Licensed Masters Social Worker
