Social Worker will oversee and facilitate intakes and admissions of new patients to the Substance Abuse Treatment Program and will ensure that the expected census is maintained.  This position will comply with the mission statement of Montefiore Health System, and the Code of Ethics of the National Association of Social Work.



•             Facilitate intake and admissions of new patients in program to ensure growth and

maintenance of census

•             Address all inquiries and referrals for new admissions

•             Schedule and complete intake evaluations. 

•             Track, and document in compliance with the Office of Addiction Services and Supports,

the Department of Health and the Joint Commission.

•             Audit all newly admitted charts to ensure all intake documentation has been completed

•             Provide outreach and education to community agencies to build referral sources

and interest in the program. 

•             Assess and manage risk and crises that may arise in the clinic

•             Facilitate temporary medication requests for patients referred from other programs

•             Respond to Epic referral work queue and conduct phone screenings for individuals on

work queue as needed

•             Provide support for program staff in absence of clinical supervisors

•             Coordinate transfers from other Opioid Treatment Programs within the Montefiore system.

•             Identify need and provide referrals for psychiatric or medical services upon admission

•             Other duties as assigned



•             Master’s Degree in Social Work from a school of Social Work accredited by the Council of

Social Work Education

•             LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker) preferred

•             Must demonstrate a knowledge of substance use and psychiatric risk assessment

•             Must have some experience with management of spreadsheets (i.e., Excel)

•             Proficiency in Spanish language preferred
