SOCIAL WORKER MSW

BRONX, NY, US


Reporting to Women's Health Senior Leadership the Social Worker contributes as a professional member of an interdisciplinary team working in an outpatient setting, provides a range of direct and indirect services for patients/families including, but not limited to, the identification and assessment of psychosocial problems and their resolutions including referrals to appropriate individuals, organizations and programs sponsored by Montefiore Medical center, Bronx Borough, New York City and New York state related to the Women's Health population. Working on the Robin Hood Grant and The Fourth Trimester while overseeing the success of this clinic hinges on the presence of a Outreach Representative/Community Health Worker (CHW) in the care team. The CHW uses the Social Worker as a referral for escalation. The Social Worker complies with the Mission Statements of Montefiore and Social Services, and the Code of Ethics of the National Association of Social Work. Incumbent will be utilized as needed at all Women’s Health OB/GYN locations within NYC and Westchester.
 

Requirements

  • Master's Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education.
  • New York State Licensed Master of Social Work required (LMSW) or (LCSW) Licensed Clinical Social Worker or limited permit, if employed for a period less than 6 months.
