Within an Integrated Behavioral Health Model of Care, the Licensed Social Worker (LMSW or LCSW) will provide clinical treatment to patients and families who are experiencing various mental health disorders including depression and anxiety.  The Social Worker’s major goal is to help patients modify the impact of these problems and assist patients with identifying effective coping mechanisms and lifestyle changes.


Responsibilities: 
•    Utilize Evidenced-Based Assessment tools and Treatment methods that are based on research and evidence gathered in clinical assessments, evaluations of outcomes and practice
•    Collaborate with an Interdisciplinary Team comprised of Primary Care Doctors, Psychiatrists, Nurses, and other internal and external professionals to improve the quality of patients’ lives
•    As needed, the Social Worker will also provide patients with assistance with Social Determinants of Health (concrete services) and make appropriate referrals to ensure patients’ safety at home and in the community

•    Annual assessment
•    Fit test
•    LMSW

Hours: 8:30a – 5:00p M-F
 
Education:
•    Master of Social Work
•    LMSW – Licensed Master Social Worker

Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek.
