The Home Care Senior Occupational Therapist assesses, plans, coordinates, implements and evaluates plans of care for clients in their home, in accord with professional standards, regulatory guidelines and agency policies.

 

Day-to Day Responsibilities:

 

  • Conducts a comprehensive assessment, including physical, emotional, and mental status for clients of all ages

  • Develops plans of care with realistic outcomes, based on interpretation of  assessment data and consideration of clients preferences

  • Designs discipline interventions according to agency policies

  • Evaluates the results of clinical interventions to measure progress toward goals

  • Reports all pertinent information about client status and progress to appropriate multidisciplinary health team members  

 Minimum Requirements 

  • Graduate of a program in Occupational Therapy approved by the American Occupational Therapy Association

  • Current New York State Licensure 

  • Registration with the American Occupational Therapy Association

  • Two (2) years at senior level in field or acute medical hospital

  • 2 years' supervised clinical experience in a medical setting preferred or equivalent combination of education and experience

  • Must have a valid driver's license and ability to provide own transporting during working hours for the purpose of making home visits
