TRANSPLANT COORDINATOR

The Department of CT Surgery seeks an LVAD Coordinator to join their team.  In collaboration with physicians and multidisciplinary team, this role will be responsible for the planning, evaluation, coordination, implementation and the facilitation of all facets of care for the patients and families requiring mechanical cardiac assistance and transplantation.

 

 

Responsibilities include:

 

  • Coordinate and provide education to ventricular assist device patients and families.
  • Coordinate the diverse inpatient and outpatient clinical management of the ventricular assist device population.
  • Ensures adherence to protocols and program certification requirements.

 

This is a Day position 7:30am-6pm

 

 

Education/Skills required:

 

  • Bachelor's Degree required; Master’s degree preferred
  • New York State License for a Registered Professional Nurse and a current registration. Bachelor of Science in Nursing.  Master's Degree preferred.
  • Or Current New York State Nurse Practitioner specialty certification and registration
  • Or Current Certification by the National Commission of Certification of Physician’s Assistants
  • Three (3) years of clinical experience in a highly specialized complex care program preferred
  • Experience in transplant patient care role preferred
  • Current American Board of Transplant Coordinator Certification after 2-3 years of practical experience preferred.
  • Clinical experience in a highly specialized complex cardiac care program preferred

Shift: Day Scheduled Hours: 7:30 AM-6 PM Req ID: 220374 Salary Range/Pay Rate: $122,100.00 - $162,800.00
