The Clinical Transplant Coordinator/Practitioner, in collaboration with physicians and other health care providers, is responsible for  coordinating the clinical aspects of care for patients  and families undergoing the process of organ transplantation.  Demonstrates advanced and diverse clinical knowledge and skills in the capacity of clinician, educator,  collaborator and patient advocate to provide both direct and indirect patient care in a semi-autonomous, self-directed role in accordance with professional practice standards and clinical expertise.  Coordinators who are Physician Assistants or Nurse Practitioners will, in addition, be responsible to their supervising physicians for clinical matters as required by state and hospital regulations. This position will be responsible for management of liver transplant pre, post and hepatology.



Master’s Degree

Current certification and registration as a Nurse Practitioner with NY state OR current license and registration as a Physician Assistant in NY State

A minimum of 2 years of clinical experience in a highly specialized complex care program.

Transplant experience preferred.

Effective English communication skills, both written and oral

Ability to use data entry and retrieval system.

Spanish preferred
