TRANSPLANT COORDINATOR PRACTITIONER

BRONX, NY, US


 

In collaboration with physicians and multidisciplinary teams, will be responsible for the planning, evaluation, coordination, implementation, and facilitation of all facets of care for the patients and families requiring mechanical cardiac assistance and transplantation. Coordinates and provides education to ventricular assist device patients and families. Coordinates the diverse inpatient and outpatient clinical management of the ventricular assist device population. Participate in "on-call" responsibilities with the cardiac transplant coordinators. Ensures adherence to protocols and program certification requirements.

 

Requirements:

NY State License for Registered Professional Nurse

NY State RN Registration

BSN Degree or Masters in Nursing/Business/Health Care preferred

NY State Nurse Practitioner License and Registration

American Board of Transplant Coordinator Certification

2-3-year practical experience preferred

Experience in transplant patient care role preferred

Effective English Communication skills, both written and oral

Ability to use data entry and retrieval system
