Group Attachment Based Intervention (GABI) is an intensive clinical service designed for families with birth to three-year old children who are receiving Preventive Services. GABI has expanded from its one site location to 6 sites city-wide (Bronx, Brooklyn, Manhattan, Queens, Staten Island). The position requires travel to various sites for training.  As an integral member of the GABI team, the Family Health Worker (aka Community Health Worker - CHW) will conduct outreach to improve patient care and community health outcomes with specific assigned target populations. The FHW will conduct public education activities in the community through indoor and outdoor community events to raise awareness about behavioral/health issues and facilitate access to the Trauma Services programs.  This position will help community members develop health management plans, goals and follow up to track progress and help meet their goals.  He/she will motivate them to be active, engaged participants in their health. The FHW will report to the GABI Clinical Manager and Clinical Administrator for Trauma Services. GABI experience preferred.

High School diploma/GED. Experience in the community with systems which impact the lives of parents and young children. Experience with the GABI program preferred. Spanish speaking preferred but not required. Position involves community-based activity in patient homes.  Experience/ knowledge of the GABI therapeutic model preferred.
